Job Responsibilities
- Actively participate in patient care rounds with other members of the healthcare team
- Assist medical staff with the selection of drug therapy and patient monitoring
- Review and interpret medication orders for clarity, accuracy, rationality, compatibility, feasibility and suitability for the patients specific needs
- Assume responsibility for patient specific pharmacotherapy plan to ensure optimal outcomes
- Enter and process medication orders whenever scheduled to do so by the Head of assigned section or whenever the need arises
- Determine workload priorities, verify medication orders, check the final product for accuracy, and assure timely delivery of medications to the patient care area.
- Act as drug information and education resource for patients, students, colleagues, medical and nursing staff and other healthcare professionals
- Conduct daily systematic medication reviews for patients in other floors other than the floor covered by the clinical pharmacist, whenever assigned
- Document therapeutic recommendations in the patient’s medical record and pharmacy intervention forms
- Detect analyze and report adverse drug reactions
- Assure adherence to the departmental policies and procedures and provide monthly reports on clinical activities and other relevant workload statistics
- Participate in the departmental performance improvement, educational and other clinical activities
- Follows all Hospital related policies and procedures
- Participate in self and others’ education, training and development, as applicable
Job Requirements
- Pharm-D / Doctor of Pharmacy
- Bachelor of Science in Pharmacy and a Pharma / Doctor Of Pharmacy
- Minimum Two (2) years of experience as a Clinical Pharmacist in the specialty for which applying (Oncology or ICU) in a tertiary hospital setting.
- Hospital Clinical Pharmacy residency of a minimum one year duration is a must
